General Instructions

The following areas need to be included for each episode of care:

1. Patient scenario/information.

   Please de-identify the patient and all organisations and persons.

   For example: Ted (do not use real names) is a 6 year 2 months old boy who was seen at the Billabong (do not use real names) CP clinic on 23/04/02/2022.

2. Summary of patient assessment process and findings
3. Review care provided against available evidence informed standards, guidelines, care pathways, research, outcomes, courses, etc.

   You might reflect on whether or not these were followed, what went well and what might be done differently in the future.

4. Resources informing care (where relevant)

   Add specific details about the current resources you use to inform patient care processes.
